When Is a Baby Tooth Extraction Necessary?

We recommend extraction only when keeping the tooth would cause more harm than removing it.

1. Severe Tooth Decay

When a cavity is too large for a dental filling or stainless steel crown, the tooth may be beyond saving. Leaving it can cause pain, infection, and damage to the permanent tooth below.

2. Infection That Won’t Go Away

If decay reaches the nerve and causes an abscess, we first try a pulpotomy (baby root canal) to save the tooth. But if the infection keeps returning, extraction may be necessary to protect the permanent tooth bud.

3. Trauma or Injury

A fall can break a baby tooth below the gum line. If the tooth cannot be saved, extraction prevents future pain and infection.

4. Over-Retained Baby Tooth

When a permanent tooth grows in behind a baby tooth that won’t fall out (“shark teeth”), removing the baby tooth allows the adult tooth to move into correct position.

5. Orthodontic Reasons

If a baby tooth blocks a permanent tooth or causes severe crowding, early extraction can create space—often part of early orthodontics planning.

What If We Don’t Extract a Tooth That Needs It?

Leaving a problematic baby tooth can lead to:

What to Expect During the Extraction

Step 3: Quick Removal – A gentle lift, and the tooth comes out in seconds.

Step 4: Gauze – We place gauze over the socket. Your child bites down for 2-3 minutes to stop bleeding.

Total time: Usually 10-15 minutes.

Throughout, we use the “Tell-Show-Do” method—explaining everything in simple, friendly language before proceeding.

What About a Space Maintainer?

If your child loses a baby tooth too early (especially a back molar), neighboring teeth can drift into the space, blocking the permanent tooth. To prevent this, we may recommend a space maintainer – a small appliance that holds the gap open until the adult tooth is ready. It’s comfortable, easy to clean, and stays for 1-3 years.

Aftercare: What Parents Need to Do

First 24 Hours

✅ Keep gauze for 20-30 minutes
✅ Soft foods only (yogurt, mashed potatoes, smoothies)
✅ Ice pack on cheek (15 min on/off) if swelling
✅ Rest – no running or rough play

❌ No sucking – straws, pacifiers, sippy cups, or bottles for 2 days
❌ No rinsing or spitting for 24 hours
❌ No hard, crunchy, or sticky foods

Day 2 Onward
  • Gentle rinsing with warm salt water after meals
  • Brush other teeth carefully, avoiding the site
  • Return to normal foods as comfort allows
Pain Management

Mild discomfort is normal. Children’s paracetamol or ibuprofen (as per dosage) usually helps. Numbness wears off in 2-3 hours.

When to Call the Dentist

Contact us immediately if you notice:

  • Bleeding that doesn’t stop after an hour
  • Severe pain that medicine doesn’t help
  • Fever or chills
  • Swelling that gets worse after 2 days
  • Pus or foul smell from the socket
